Extra work may be required on the supporting cartilage structure of the nose, due to the trauma caused to the nose in open rhinoplasty, and generally more swelling occurs than in the closed technique. Open rhinoplasty is sometimes favoured by plastic surgeons because it enables them to see the cartilage in its natural position, making it much easier to see exactly what work needs to be done and carry out the procedure more accurately. This is especially important in more complex operations, for instance open rhinoplasty is used almost exclusively in ethnic rhinoplasty where procedures are often more problematical. It is more straightforward to find a surgeon who will be able to carry out open rhinoplasty well. As open rhinoplasty offers the best training opportunities to medical students, newly qualified surgeons are more likely to be proficient in the open technique than they are in the closed. There are plastic surgeons that have had many years of experience and are skilful enough to carry out more complex procedures using the closed method, but they may be harder to come by.
